Information collected is kept confidential and will help determine which health issues are of most significance today and help guide health research funds.  Refer to Gordon Setter News issue 737, September, 2015 for additional information. Your participation will greatly benefit our breed. Remember to contribute information on your healthy dogs as well

 

 

            This survey will be analyzed and recorded by the Orthopedic Foundation for Animals (OFA). All information submitted is confidential and numbers are used only in aggregate.

            Please answer the questions to the best of your ability and use a separate survey form for each Gordon Setter you have owned, living or deceased. As you complete a survey for each dog, simply restart a new one for the next dog you are reporting.

            In order to complete the survey you will need information regarding your dog's health. If needed, your veterinarian will be able to supply you with a list of issues your dog has had diagnosed, so that you are able to use the diagnosis listed under each category. Whenever possible, please use a veterinary diagnosis. There is a box under every category where you can answer “Other” and add a response or diagnosis if it is not listed.

            You will need to indicate which vaccines your dog has had, and what flea/tick and heartworm preventative you use. You will be asked what health clearances your dogs have, so you may need to have your certificates at hand for each dog.

            This is a very comprehensive survey. It should only take about 15 minutes/dog, but may take you as long as 20-30 minutes to complete, especially for the first dog you submit. In many instances, when you answer "yes" there are other screens that will appear with additional related questions.

            Please enter complete information for deceased dogs, and current information for any dogs that are living. In the future, you can go into a new survey and only enter new information (updates) for those living dogs that you had previously entered, which will not add to the total number of dogs, but will add to the existing information in the database.

            ALL Gordon Setter owners are invited to participate in order to make this survey as valid, valuable and complete as possible. If you have acquaintances who are not GSCA members, please share this link with them.
